John Terry had to be stretchered off after just 15 minutes of Chelsea's crucial Premier League finale against Bolton on Sunday.
Terry appeared to land awkwardly after colliding with goalkeeper Petr Cech and Bolton striker Kevin Davies in the 10th minute.
The England international appeared to be in pain and distress and slapped the turf with his hand to indicate his pain.
Terry was replaced by Juliano Belletti in the Chelsea defence and the skipper was taken to hospital for tests on the injury.
The injury means Terry must be regarded doubtful for the Champions League final against Manchester United in Moscow later this month.
